I haven't fully landed on a final specification, but to begin the
process: any thing with a octothorpe can be ignored _for now_.

re-write Makefile

it now assumes GDC by default. this can be changed with the DC
environment variable (you should also change the DFLAGS variable
as well). this will make it somewhat simpler to maintain in the
future.

also included in the addition of manpage installation.

update Makefile and README to reflect GDC change

since the xdg.concat function now works with GDC, we don't need
the special comments.

re-write xdg.concat so it works on GDC.

it now doesn't use varargs as none of the calls to concat were more
than two parameters (i.e. the base string, and the one to append).

not concerned about the API since it's a private function.
